How Are Different GLP-1 Meds Used Today?

Blog Image: How Are Different GLP-1 Meds Used Today?

“A patient’s health condition, lifestyle preferences and prescriptive goals may benefit from different formulations.”

While the variety of GLP-1 prescription medications mimic the gut hormones that chemically signal regulation of blood sugar spikes by curbing appetite, each formulation can vary in active ingredients, dosing requirements, delivery methods, and may offer slightly different benefits. Moreover, FDA-approved indications vary with some GLP-1s recommended for managing type 2 diabetes, while other formulations are explicitly approved for chronic weight management to treat obesity or overweight individuals with key metabolic conditions.

Although all glucagon-like peptide-1 drugs aim for similar outcomes, they can differ in how they are administered as well as how long the reduction of appetite will last. In general, GLP-1 medications depend specifically on the peptide molecule’s half-life (short-release vs. extended-action). This highlights the role of the professional provider who understands the numerous criteria to consider, such as the biological pathways they engage as receptor agonists. In most cases, the GLP-1s you’ve heard about require weekly injections.

Can Tirzepatide outperform Semaglutide in clinical trials?

Single GLP-1 RAs activate and bind to exactly the same receptors in the gut, pancreas and brain that naturally-released glucagon-like peptide-1 hormones act upon. But, to control metabolic processes, single GLP-1s only focus on a single incretin pathway, while dual GLP-1 / GIP agonists double down and activate a second pathway for glucose regulation. GLP-1 reduces blood sugar by suppressing a hormone that signals the liver to release stored glucose into the bloodstream. GIP, in contrast, stimulates insulin release in response to glucagon levels to produce more significant effects on weight loss and metabolic health for some patients.

Can doctors prescribe once-daily oral GLP-1 meds?

Yes, there are options for prescribing oral GLP-1 formulations based on the specific indication for treating high blood sugar or chronic weight management. Currently, Rybelsus is an oral semaglutide approved for treatment of type 2 diabetes, Wegovy now offers an oral semaglutide approved for weight loss and reduction of cardiovascular risk, and Foundayo (orforglipron) is the first non-peptide GLP-1 receptor agonist approved to treat obesity as a once-daily oral medication. Developed by Eli Lilly, it is the first small-molecule GLP-1 that is easily digested and can be taken any time, with or without food.

Can doctors prescribe sublingual GLP-1 meds?

Yes and no. Although the FDA has approved several oral GLP-1 prescriptive medications that are swallowed as pills, there are no FDA-approved sublingual GLP-1s that are clinically proven to be a needle-free alternative to injectable GLP-1 therapies. While you may see ads featuring sublingual drops or disintegrating oral tablets, these are compounded pharmaceuticals that are a custom-mixed formulation that has not been approved the FDA for safety, efficacy nor bioequivalence as a glucagon-like peptide-1 receptor agonists. While your doctor has broad authority to prescribe drugs off-label, most treating obesity do not prescribe unapproved peptides due to health risks like dosing uncertainty.

Can doctors prescribe chewable Tirzepatide gum?

Maybe or maybe not. Tirzepatide is officially FDA-approved for treating type 2 diabetes or weight management for obesity as an injectable peptide medication. Nonetheless, some licensed compounding pharmacies and online telehealth platforms, such as The Wellness Company, do market and prescribe both compounded oral tabs and sublingual tirzepatide gum. However, compounded medications are not evaluated by the FDA and major medical groups express the extreme need for caution when using them. These options have dosing uncertainties, efficacy issues, safety concerns, and may not be cost-effective alternatives.

Compounded GLP-1 versus Brand Name Medications

Although multiple brand-name semaglutide and tirzepatide options are commercially available, specialized compounding pharmacies can also mix customized ingredients to produce variations of GLP-1 medications. With that said, these compounded prescription medications are not FDA-approved in the same way as the popular brand-name GLP-1 drugs. However GLP-1 compounded medications do offer greater flexibility in dosing and are often more accessible during shortages of brand-name drugs and might cost less as well.

While naturally-released glucagon-like peptide-1 and pharmaceutically-produced receptor agonists act on the GLP-1 pathway for regulating metabolism, only a handful of brand-name drugs have FDA-approval for use in type 2 diabetes and obesity management. In addition, where all GLP-1 drugs can reduce appetite and influence metabolism, only brand-name semaglutide and tirzepatide are FDA-approved for weight management. However, some that have been approved for controlling diabetes may also lead to weight loss as a secondary effect.

MRC medical weight loss plans are highly structured programs that are supervised by healthcare and weight management professionals to help individuals achieve safe and effective weight loss. In fact, our goal is to simplify access to compounded weight loss medications that are tailored for your body’s unique needs. If our provider confirms compounded drugs are suitable for you, they will prescribe them accordingly to be filled at an authorized compounding pharmacy. These have shown remarkable effectiveness with minimal side effects, making them a clear choice for some clients at MRC Fort Collins.
